FOR SUBMISSION BE FORM (INCOME FROM SALARY)

1. EA/EC Form
2. Statement of insurance (life, medical and education-if any)
3. Receipt of expenses ( book, medical for tax payer or parents, sport equipment, educational fee and etc)
4. Receipt if u buy computer and also pay broadband subscription fee
5. Birth certificate of your children 
6. Your child details ( D.O.B, school, college, university, married or not, disable or not)
7. Zakat and donation receipt (if any) 
8. any other income that u received in the whole year assessment
